Eficacia de la terapia osteopática manipulativa en casos de cefaleas tensionales

La cefalea tensional es una de las afecciones médicas más frecuentes e incapacitantes. Afecta al 15% de la población general, generando altos costos socioeconómicos globales, y las opciones actualmente disponibles de tratamiento son inadecuadas. A pesar de existir revisiones previas, se considera necesaria una puesta al día para ofrecer una visión lo más actual posible del abordaje fisioterápico de la cefalea tensional. Los objetivos de este estudio consistieron en comprobar la eficacia de la terapia manipulativa en pacientes con cefalea tensional, destacar la elección de la terapia manipulativa ante otros tratamientos de fisioterapia y farmacológico y desestimar el uso de farmacología antiinflamatoria, ante la terapia manipulativa. Se realizó una búsqueda de artículos de revisión sistemática y estudios realizados en el período 2010-2020, en las bases de datos PubMed Central y en Google Academy. Para la búsqueda se utilizaron palabras clave como cefalea tensional, tratamiento osteopático manipulativo, dolor cervical y dolor de cuello. Se incluyeron artículos que hacen referencia tanto a manipulación vertebral como craneal. La mayoría de los estudios fueron revisiones sistemáticas, estudios comparativos o pruebas pilotos. Los artículos revisados apoyan la noción de que los pacientes con cefalea tensional pueden beneficiarse con el tratamiento osteopático manipulativo. Se pudo corroborar que el tratamiento osteopático manipulativo reduce la frecuencia e intensidad de las cefaleas, así como también, posee resultados más prolongados y sin efectos adversos ante el tratamiento farmacológico. Los resultados de esta revisión sistemática muestran que hay suficiente evidencia científica a favor de la aplicación del tratamiento osteopático manipulativo para el manejo de la cefalea tensional, sin desestimar técnicas más convencionales, como por ejemplo elongación y masoterapia.
